W. Va. Code § 18B-1-6
Each entering student must submit a completed health form(s) provided and approved by 91Ï㽶ÊÓƵ prior to matriculation.
Students must provide documentation of all vaccinations and where indicated proof of immunity as listed in the Procedure for Institutional Policy DO-03, unless medically contraindicated.
Students must comply with all immunization requirements and timelines set forth in the Procedure for Institutional Policy DO-03.
Students must comply with any current vaccination and testing policies of any affiliated hospitals or other healthcare facilities/providers with which the student may participate as part of any clinical rotation or other curricular activity. Students should be aware that such policies may be changed at any time.
